How easy would it be to code non-weapon combat? As in hand to hand combat?

Adding damage to Null/Hands weapon: easy
Adding animations for it: almost impossible

As far as I remember there are no build in punching/kicking animations available and we can't add new ones. That why I say it is almost impossible. The only two options to do that is switching from mdr to md3 player models (huge increase in size ... also rewrite of emote system and other things, all animations would have to be redone) or switching to a new model format (for example iqm ... also huge rewrites coming, no compatibility to vEF/ioEF/ioCow, all animations would have to be redone).

Ah right that sucks.

How easy is it to rewrite the health system for RPG-X? Maybe instead of having a "Healthy" bar, have an actual bar that decreases? And have a picture of a heart, and when it decreases, the heart slowly breaks. Or maybe have a heart monitor (like in hospitals)

Health Bar is certanly possible. It has been originally that way in ef. However considering that If I'd have to check my body for injuries I could merely assess the criticality of injuries I think the current UI fit's the Roleplay-Purposes best. Maybe we can reintegrate that as an optional UI-Element but taht is entirely gsios area.
